REGISTRATION INFORMATION

Families new to the district should register at the main office of each school.

Proof of Residence:

Maercker School District 60 requires a photocopy of three (3) of the following items as evidence of residency:  Driver's license, home ownership title or deed, current lease showing the building owner's certificate of residency, voter registration card, utility bills (water, electric, gas) within the past three (3) months, State identification card, Public Aid card, most recent property tax bill, and automobile insurance.  In addition, a three-page questionnaire must be completed by the parent or legal guardian of the student(s). 

For each subsequent school year, the verification of residency must be demonstrated annually by providing a photocopy of two (2) of the documents cited above and by completing the questionnaire.

If information received at the time of registration does not satisfactorily confirm residency, District 60 may take any or all of the following steps:

  • Request additional verification of residency
  • Contact realtor or lending institution
  • Contact the Village or City government
  • Request a personal home visit

If District 60 determines non-residency, the parent/guardian will be:

  • Notified of the student's immediate removal from school, subject to the appeal procedures outlined in Board Policy 7:60
  • Charged the current tuition fee per child per day of attendance
  • Turned over to the authorities for legal action

If the parent/guardian does not remit tuition costs, District 60 may take any necessary legal steps to collect the tuition costs.

Birth Certificate:
A certified copy of the birth certificate must be presented within 30 calendar days of the enrollment date for children entering kindergarten or children new to the district.  The police will be notified if the birth certificate is not presented within 30 calendar days.

Physical Examination, Dental, Vision & Immunizations:
Kindergarten, first grade children who did not attend kindergarten, all out of state children new to the District and all sixth grade children are required, by law, to have a physical examination and be properly immunized. In addition, children entering kindergarten, first grade children entering school for the first time, second grade children, and sixth grade children will be required, by law, to submit a dental examination form to the school. (The physical and vision examination must have been completed within one year prior to the child entering school.) The District requires the physical and vision examination and immunizations be presented to the school officials two weeks prior to the first day of school. Failure to comply with the physical examination and immunization requirements will cause the child to be excluded from school. 
A vision examination, by a licensed physician or optometrist, is required upon entry into Kindergarten (or entering school for the first time).  Failure to comply with the dental and vision examinations will result in withholding the child's report card.

The DuPage County Health Department provides immunizations for those children who have no insurance, have insurance that does not cover immunizations, or are Medicaid/All Kids recipients.  A fee is required per immunization.  Call the DuPage County Health Department (630) 682-7400 to schedule an appointment .  The DuPage County Health Department is located at 422 N. Cass Avenue, Westmont.

If you need health insurance for your children, applications for "All Kids" are available in the nurse's office, the DuPage County Health Department, or available on line at http://www.allkidscovered.com/ .  Families who are in need of financial assistance, for health care, may call Access DuPage (630) 510-8720 to determine eligibility.

Dental assistance may be obtained by calling the DuPage County Health Department (630) 682-7400).

Athletics:
Boys and girls who participate in inter-school athletics will be required to submit evidence from their physician that they are physically fit. Forms are available and can be picked up in person at the District 60 Administration building or individual schools. These completed forms must be on file at the school before the student athlete will be permitted to practice.
