Health Requirements for 2024-25

Welcome to Maercker School District 60! Listed below are the required State of Illinois health documents for entry into Illinois schools. In addition, if your child requires medication administered during the school day, the district's medication authorization form is required. When a student requires daily or regular medication, parents must make every effort to give prescribed doses of the medication at home. Therefore, only medications (prescription, non-prescription and herbal) prescribed by a physician and which are essential for the student to remain in school shall be given.

The State of Illinois Certificate of Child’s Health Examination/physical form is due October 15 of the current school year. Your child may be excluded from school starting on October 15 of the current school year until all forms have been completed and turned in, or if you provide a signed notice from your child’s physician of the scheduled appointment date.

HEALTH EXAMINATION/Physical for Pre-K, Kindergarten, sixth, ninth, and new transfer students from out of state will need a health examination turned in by October 15, 2024. New students to the district from out of state have 30 days from the first day of school to submit a health exam. Health examinations shall be conducted within one year of the start of school for students.

EARLY CHILDHOOD/Pre-K
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history completed and signed by parent. Physical exams are due within 30 days from the start date.

KINDERGARTEN
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history completed and signed by parents.

Eye Examination must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15 of the school year

Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.

2ND GRADE
Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.

6TH GRADE
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history completed and signed by the parent.

Proof of one dose of Tdap/DTaP vaccine and one dose of MCV4 (meningococcal conjugate vaccine) vaccine on or after the 11th birthday

Dental Examination must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.

NEW STUDENTS FROM OUT OF STATE
Physical Examination must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history completed and signed by the parent.

Physical exams are due within 30 days of the first day of school. If an exam is not submitted within 30 days, the student may be excluded from school per Illinois Law. An out of state health exam that is comparable to the Illinois Health Examination requirements, may be accepted at the time of first entry into an Illinois school.

Vision and hearing screening is mandated per IDPH & ISBE for transfer students from out of state. Please put in writing to the school nurse if you do not want your child screened for vision and hearing.

STUDENTS TRANSFERRING FROM OTHER ILLINOIS SCHOOLS
Transfer of current Illinois school health records (physical exam with up-to-date immunizations, dental exam, and eye exam).

School health requirements for grade level as noted above.

SPORTS PHYSICALS
If your child will be trying out for any sport, they must have a sports physical on file prior to try-outs. A school physical exam may be used in lieu of a sports physical, HOWEVER, a sports physical CANNOT be accepted as proof of a physical exam for a mandated grade level physical exam. School physical exam (Certificate of Child Health Exam Form) valid for 365 days from date issued and Sports physical (Sports exam form) valid for 395 days from date issued. *Tip: Have the Dr. complete both the school physical exam form and sports exam form and you’re covered for school and sport requirements!

WAIVERS
Medical Objection: A statement from the MD, DO, APN or PA indicating that an immunization is medically contraindicated is acceptable and needs to be attached to the student’s physical exam form. In case of a disease outbreak, the student may be excluded from school for 14-21 days.

RELIGIOUS OBJECTION: Religious exemption for immunizations requires an Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations Form completed by the parent and the student’s healthcare provider, and must be within one year prior to the first day of school. In case of a disease outbreak, the student may be excluded from school for 14 to 21 days of school.

NECESSARY MEDICATIONS
Any over the counter medication not prescribed by a doctor such as cough drops, Tums, and Tylenol and any prescription medication must be brought in by an adult in the original packaging, with the student’s name, name of medication and dosage. Students will not be given prescription and/or over the counter medication without a completed Medication Authorization form which has been signed by the parent/guardian and the health care provider.

A Student Self-Administration of Asthma Medication Consent Form must be completed by the parent/guardian and a copy of the original box/prescription must be kept on file. An Asthma Action Plan (AAP) is available for students with asthma. Maercker School District #60 requires an Asthma Action plan for students with asthma.

The Illinois Food Allergy Emergency Action Plan and Treatment Authorization form is available for students who have food allergies and may require necessary medication during the school day. The form needs to be filled out and signed by the parent/guardian and student’s healthcare provider.

VISION AND HEARING SCREENINGS
Vision and hearing screenings are provided annually to District 60 students at the State-mandated grade levels, as designated by the Illinois Department of Public Health. All students in special education programs, students transferring into the District who have not previously been screened, and students to be tested related to classroom teacher requests are screened during this annual program.

Scheduled intervals for Vision and Hearing screenings are as follows:

Vision screenings

  • Preschool, Kindergarten, 2nd and 8th grades

  • Special Education students (includes students in speech)

  • Foster care and transfer students from out of state

Hearing screenings

  • Preschool, Kindergarten, 1st, 2nd and 3rd grades

  • Special Education students (includes students in speech)

  • Foster Care and transfer students from out of state

  • Students having known or suspected hearing loss
